2. Fill up the form:

Remember to include your school details and particulars of claim.

3. Attach the receipts on a A4 paper

Number the receipts according to what is listed in the excel file. Would appreciate if you could assist us by including your school name as well.

Submit the hard copy completed form (with receipts attached at the back) together with your project report.

All projects must reach SKM's office by 16 July, 2010 (6pm)
